Background Check Information
  Background Check Info - As a contracted agency with third-party state and county funding sources (such as Children’s Long-Term Support, IRIS and others), SMILES must conduct background checks all staff and volunteers. To remain in compliance with these agencies, please provide your Name, Social Security Number, and Date of Birth. This information will only be used for this purpose. Third-party agencies provide approximately 66% of our program revenue.
