Please have the following REQUIRED information handy prior to starting athlete registration (you have 20 minutes to complete this form):

• Current USSA #
• Athlete health insurance information (company name, address, telephone number, policy #)
• Please read the SVSEF Program Agreement and Code of Conduct. You will be required to electronically sign these agreements for your athlete on the next screen to complete registration.

 

 

The tuition amount shown above is the total amount of tuition due for that team. Fifty percent (50%) of tuition must be paid to SVSEF by September 10, 2013 and the remaining 50% must be paid in full on or before November 10, 2013. A discounted tuition rate of 25% off the combined total is available for athletes participating in two teams simultaneously and will be reflected on your SVSEF statement. Baldy Ski Passes will be due in full on or before November 20, 2013. Cost is determined by Sun Valley Company but paid to SVSEF.
