Prepare Affair Volunteer Form

Join us for Prepare Affair in November 2023 to rake leaves and clean gutters for seniors and homeowners living with disabilities in your community. While we'll be celebrating Prepare Affair 2023 on November 18th, groups are invited to schedule any date in November that works best for their schedule.

PWC and its volunteers will follow the CDC’s recommended guidelines for COVID-19 safety, as well as all local public health guidance. As this guidance changes, PWC will keep volunteers updated.

By submitting this form:

You hereby release and hold harmless People Working Cooperatively, Inc., their funders, supporters, officers, agents and employees and the homeowner for whom I am doing home repairs for any injury which I may suffer or incur as the result of my volunteering.
You understand that PWC takes the privacy of its clients very seriously and will never breach a client's privacy or the confidentiality of a client's record.
You understand that volunteers are covered up to $1,500 in medical insurance related to their specific volunteer duties.
You allow PWC to use your image/photograph for agency promotional purposes.

Please note, these waivers and releases shall remain in effect for all PWC volunteer activities.
